<p>Leicester City Council (the Authority), on behalf of our local systems partners in Leicester, Leicestershire and Rutland, is leading the commissioning review of the discharge to assess Pathway 2 arrangements for Leicester and Leicestershire to ensure that our performance as a system continues to deliver good outcomes for people discharged from an acute setting. Our long-term aim is to have a robust and coherent discharge to recover, then assess model in place that can deliver good outcomes for those requiring a period of bed-based reablement and recovery. This compliments the significant amount of work being done around Pathway 1 as part of our Home First offer.</p>
<p>The discharge to assess model is based on four clear pathways for discharging people. The definition of Pathway 2 is given below.</p>
<p><strong>Pathway 2 definition:</strong></p>
<p>The principles of Pathway 2 are recovery, rehabilitation, assessment, care planning or short-term intensive support in a 24-hour bed-based setting, ideally before returning home.</p>
<p>The focus of that stay should be on enabling independence which supports the ideal outcome for the individual which is a return home.</p>
<p>This facility is most likely going to be based in a care home, a rehabilitation facility or a hospice and we are seeking responses from all sections of our market who feel they can offer intermediate care services in line with the principles set out above.</p>
<p>To ensure we promote the principles of a home first approach and to best support those needing a Pathway 2 offer, commissioners are keen to explore what the intermediate care model should look like. There are a number of elements that we would like to test out including how we support people with more complex needs at the point of discharge from hospital.</p>
<p>Further to the Soft Market Testing recently undertaken by the Authority as part of the commissioning process, the commissioners of Discharge to Assess (D2A) Pathway Two Review are keen to hold an Online Market Engagement Event.</p>
<p>The Online Market Engagement Event is anticipated to be particularly focused on the Pathway 2 model (which are also referred to as intermediate care services).</p>
<p>This is an opportunity for organisations interested in tendering for these services to work with commissioners to inform key elements of the model.</p>
<p>There are two similar events, each event will provide opportunities for organisations to discuss their thoughts around:</p>
<ul>
<li>The model for P2 and the capacity we need for services and how other NHS and social care services will support the model we need for Leicester and Leicestershire</li>
<li>How we incorporate specialisms into the model to support people with more complex needs and how we achieve the right outcomes for people accessing services.</li>
</ul>

<p>The engagement event will be held remotely through Microsoft Teams. The Online Market Engagement Events are being held on Friday 25 February 2022 commencing at 10.00 a.m. to 12 noon and on Monday 28 February 2022 commencing at 10.00 a.m. to 12 noon. Interested organisations should only attend one of these events. <p>Participation or non-participation in the market engagement events will not prevent any provider from participating in any future procurement process, nor is it intended that any information supplied as part of these discussions will place any provider at an advantage in a potential procurement process. The formal procurement process is anticipated to commence later in July 2022.</p>
