The tender has been divided into two Lots.

Lot 1 - The Community Ophthalmology service is a community based provision of triage, assessment, treatmentand management of ophthalmology conditions.
The Ophthalmology services are for service users suffering from a range of minor eye conditions as listed in thespecification. The service will be provided from one location within each borough, although patients will be ableto access any services location irrespective of their registered GP.

Lot 2 - The aim of the Low Vision service is to allow people with sight loss, often elderly patients, disabled andthe young to access services easily and nearer to home.

Both services are for service users registered in the 3 boroughs. The service is to be provided via a hub and spoke model across BHR the three CCGs.

Bidders should note that the Authority has arranged for a Bidder Information Event to take place on Tuesday 27 March 2018 and for visits to be undertaken to view Loxford Clinic as a potential premises site (for Lot 1) from 27 March – 10 April 2018. Please comply with the instructions in the ITT Overview document to attend the event and / or to arrange a visit to these premises.

Bidders should also note the following with regards to completing the Online Questionnaire for this ITT: 1) Additional information and supporting documentation should only be supplied as annexes to response to questions. (2) Any annexes should be directly relevant to the particular question and should be cross-referred to, and its relevance explained, in the main body of the relevant Response contained within the Online Questionnaires. Annexes must be attached to the response to the question it relates to. Evaluators will only consider relevant attachments attached to the questions. (3) Any character / word limit given for a specific question must be adhered to. (4) Your completed Response must be submitted no later than the Deadline. The Authority reserves the right to reject any Response which is not received by the Deadline. (5) Bidders should note that cross-referencing your response is allowed within a section only. The evaluators are assigned on a section by section basis and they may not have access to your responses/attachments included in other sections.
