Introduction
Luton is a large and growing town around 30 miles to the north of London. Luton Council is a unitary Council covering an area of approximately 16.74 square miles (43.35 km2) and the current population of Luton is estimated at 216,000.
The Council employs approximately 3,000 people based in a variety of locations such as offices, schools, nurseries, crematorium, parks pavilions, community centres, sports centres, day care, residential, police station, hospital, etc. These locations are located across the Borough.
In order to support the operational process and monitor the performance and effectiveness of the NHS Health Check programme at all levels, Luton Council (LC) is procuring an information system. The solution will provide a comprehensive suite of reporting and analytical tools to support the information requirements of both local and national Health Check programme across the GP practices in Luton.

Section1: Background & Context
Luton Council has a statutory responsibility to offer NHS Health Checks to the eligible population.

The aim of the NHS Health Check Programme is to help prevent heart disease, stroke, and diabetes and kidney disease. Everyone between the ages of forty (40) and seventy four (74), who has not already been diagnosed with one (1) of these conditions, will be invited (once every five (5) years) to have a check to assess their risk of heart disease, stroke, kidney disease and diabetes and will be given. Support and advice to help them reduce or manage that risk.

The primary objective of the information system is to support the administration of the NHS Health Check programme. This will be achieved by:-
• identifying those who are eligible for an NHS Health Check;
• categorising the eligible population by their status in the Health check process and provide a ‘patient journey display’ to assist GP practices to administer the programme; and providing programme activity and outcome reports for LC.

Section 2: Service Requirement
The solution will provide the core reporting system for the NHS Health Check programme including:-
? identification of the eligible populations for Luton;
? assist GP practices to deliver the programme. The software must have a function to categorise the eligible population by their status in the Health Check process; and
provide programme activity and outcome reports for Luton Council by registered practice.

The solution will consist of the following two modules of functionality:-
? Module 1: GP Practice software to identify the eligible population, and categorise the eligible population by their status in the Health Check process; and
? Module 2: Performance and outcome reporting of NHS Health Check activity to be accessed by LC.

Both modules will be provided by one online information system.
The two (2) modules are illustrated in the following diagram:


The solution must identify the eligible population who are registered with a Luton GP practice.

At a minimum, the solution must update monthly to ensure GP practices have access to the latest available information about their eligible population and their status in the programme.

The solution must enable practices to manage call and recall of eligible patients.

The solution must support the NHS Health Check minimum dataset.

The solution must make all recorded data available for enquiry, print and analysis by system users (patient identifiable) and Public Health teams (pseudonmised / anonymised).

The solution must provide an ability to generate standard reports, based on templates provided within the solution, and custom reports.

The solution must provide tools for ad hoc analysis of the NHS Health Check data to support information needs of LBC.

The solution must support a minimum of two (2) access groups:-

? Local GP Practice – Users in this group require access to view their own GP practice eligible population and their eligible population’s status in the NHS Health Check programme; and

? Local Authority – Users in this group may have access to their Local NHS Health Check programme to review data and reports.

The current operating system has been working effectively and is in place across all Luton practices. Any new operating system would need to be in place and operating by 1st April 2020.
Section 3: Budget/Spend Information
The budget for this software is £24,000 per year with the anticipation of a one + one contract agreement. This is the total budget for the reporting element of the contract if any additionally equipment is necessary for either the GP practices or Local Authority then this should be included in this budget.
Section 4: Performance Measures
Service Levels
The core hours of business for the Service Provider are 09:00 to 17:00 on each Business Day.

The Service Provider undertakes to provide service availability for Service Users and Service Commissioners according to the following minimum standards, together known as the “Service Levels”:

Priority Description Response time Target resolution time
Priority 1 The entire Service is "down" and inaccessible. Priority 1 incidents shall be reported by telephone only. Within two Normal Business Hours. Four (4) Normal Business Hours. Continuous effort after initial response and with Council co-operation.

Priority 2 Operation of the Services is severely degraded, or major components of the Service are not operational and work cannot reasonably continue. Priority 2 incidents shall be reported by telephone only. Within four (4) Normal Business Hours. Within two (2) Business Days after initial response.

Priority 3 Certain non-essential features of the Service are impaired while most major components of the Service remain functional. Within twelve (12) Normal Business Hours. Within seven (7) Business Days after initial response.

Priority 4 Errors that are, non disabling or cosmetic and clearly have little or no impact on the normal operation of the Services. Within twenty four (24) Normal Business Hours. Next release of Software.
