Description
The Borough of Poole is advertising the following tender opportunity for the delivery of a Supported Living Service for Young People with a Learning Disability and/or Autism (aged 16-25 years) based at Woodlark House in Poole.

As part of the Council’s Big Plan 2018-21 strategy for people with a learning disability and in response to the national learning disability Transforming Care agenda and wider SEND reforms this service will support people up to nine young adults with complex needs to remain close to home and develop their skills for independence.

This is a new purpose built scheme, currently under construction, consisting of five accessible flats which have been designed to a high specification for this client group and 1 staff flat provided at a peppercorn rent. The scheme is in the Canford Heath area of Poole next to Montacute School, alongside the development of two larger general needs housing schemes also currently under construction.

Completion is anticipated between Nov-Dec 2018, with people moving in over a three month period thereafter.

The service will consist of a background support model, including the best use of available assistive technology and innovative working arrangements. This will then be enhanced as required with additional person specific care and support though each person’s Personal Budget.

This is a time limited supported living scheme to help young people to build their skills before moving on to alternative longer term accommodation elsewhere, post 25 years of age. We envisage this will be the primary purpose of the scheme.

The contract is for five years with a possible two year extension.

As this is a new service there are no TUPE implications as part of this tender.
