Description
<p>Bradford Council, as Lead Commissioner on behalf of itself and the NHS Bradford District and Craven CCG and BDCT, wishes to inform organisations of its plans for recommission of Dementia Advice and Support provision in the Bradford District.</p>
<p>This project aims to work with people accessing services and providers, to co-produce a post diagnostic support service to meet need for people living with dementia across the Bradford District, and their families and carers. We are interested to hear from providers of these services, and as part of the engagement process will work to understand barriers for organisations to be able to bid and develop an inclusive approach.&nbsp;</p>
<p>The service has the aims of:</p>
<ul>
<li>Raising awareness and reducing stigma of dementia in all localities in Bradford District; and supporting people to be diagnosed with dementia, ensuring this includes 'easy to ignore' groups such as learning disability, LGBTQ, BAME, and people with early onset dementia Improving access to health and social care</li>
<li>Supporting people to live as they wish and receive the care and support they need in their preferred setting</li>
<li>Raising awareness of opportunities to contribute to research</li>
</ul>
<p>At the current time the Council would like to establish interest in this opportunity. To be kept informed of updates on this project please register your interest.
