City & Hackney Community ENT

City and Hackney CCG are looking to commission a Community ENT provider to provide services to City and Hackney GP registered patients within our borough areas. We are looking for a service provider than can deliver services from more than one venue and is committed to providing education, training and feedback to primary care to support care in the community.

A single contract will be offered to the successful provider on a block price arrangement which may be staged in the early months until minimum activity levels are achieved.

Service:
The service is an alternative to hospital based treatments offering a one stop service for appropriate ENT assessments, diagnostics and treatments as well as supporting primary care pathways and care at GP practices.
It will have 3 main functions:
• Referral review & pathway choice
• Support/Education for primary care professionals
• Assessment & Treatment of appropriately referred patients (estimated 1100 referrals per year)

Contract Terms:
Contracting will be subject to the NHS standard contract for a period of 3 years with an option of a 2 year extension and the total estimated contract value is over £600k over 5 years.
