Description
Kent Community NHS Health Foundation Trust (KCHFT) is seeking tenders from suitable experienced providers who can deliver a Home with Support Service.
The Home with Support Service supports the discharge of patients who have been assessed in the Acute as safe to go home by receiving a further assessment of need, rehabilitation and a period of recovery at home and develop a plan of care and onward referral if required.
This service is an extension of the KCHFT Rapid Transfer of Care Service, and will work in collaboration with Rapid Response, Acute Response Team, Rehabilitation teams to support patients with complex need to be transferred home from either the Accident and Emergency Dept. (Front Door) or Wards (Back Door) from an Acute Hospital or from a KCHFT Community Hospital bed. Short term support may also be required for patients who may need a period of additional care to enable them to remain at home and avoid a hospital admission.
KCHFT is looking to work with social care providers to support the identified care needs as part of the discharge plan of care for the patient.
The initial delivery model, per block, consists of a volume of hours. Blocks will be 'single handed' for the months June through October and 'double handed' November through May' unless otherwise directed.
The Contract will run until the end of March 2020, unless terminated in accordance with the Terms and Conditions. There are two options to extend the Contract Term by 12 months (March 2021) and a further 12 months (March 2022).
Expressions of interest will only be accepted via the Trust Procurement Portal.
KCHFT is undertaking this Procurement activity under the Competitive Procedure with Negotiation and Light Touch Regime.
