<p>Rochdale Adult Care Services and the Heywood, Middleton and Rochdale Clinical Commissioning Group (CCG) are committed to reforming the Care at Home services within the Borough of Rochdale and ensuring they are equipped to meet the changing and growing demand from the population it serves.</p>
<p>We are seeking to commission a Care at Home service that is focused on delivering outcomes for individuals who live in the Borough of Rochdale. This Service specification outlines our vision for the delivery of these services, the key features of the service and the outcomes we expect it to achieve. There will be an enhanced focus on hospital admission avoidance, behaviour change and self-care.</p>
<p>This is an exciting opportunity to provide a service to people in a new and flexible way to maximise their ability to live independently and safely, in their own home within the community.</p>
<p>As part of the Greater Manchester devolution process Rochdale Borough has developed a transformation plan that aims to fully integrate health and social care services across the whole system. This includes the development of an ‘out of hospital’ model of care delivered on a neighbourhood basis which provides services that support people to remain at home. The provision of Care at Home is an important element in the development of an integrated neighbourhood approach and this specification outlines a new approach to commissioning home care services within the context of the transformation plan.</p>
<p>We have commissioned Care at Home on a neighbourhood basis aligned to the six Integrated Neighbourhood Health and Social Care teams. In addition we have commissioned a more personalised service that is person centred, maximises the use of community assets and people’s individual strengths. We want to further develop the model to deliver a broader range of services e.g. enhanced care worker roles and for Care at Home services to be fully integrated into the neighbourhood teams. Consequently, it is our expectation that the successful providers will enter into any contract arrangements with Rochdale Borough Council in the knowledge that the service will be developing and changing alongside the implementation of the Transformation Plan.</p>
<p>The Council is therefore looking to procure Care at Home services from providers with an appetite for transformation and innovation and a strong ethos of partnership working. As such, Providers are advised that the Council reserves the right to review the services to be provided in respect of this invitation to tender as required.</p>
