Complex Dependency ICT Solution for Cheshire

The Cheshire and Warrington Complex Dependency Programme has been set up from April 2015 following a successful bid for Government funding through the national Transformation Challenge Award scheme. The Programme will build on what is already in place across the Cheshire Sub Region to establish new, joined up models of service delivery across agencies to tackle the causes of crisis for children, families and individuals across a range of related and complex issues. It will also ensure that there is more support available for families and individuals that are already in a state of crisis. These interrelated issues that the Programme focuses on are: • Working with each locality on Stage 2 of the Troubled Families Programme. • Adults and children involved in crime or anti-social behaviour • Children who have problems at school • Children who need help • Children at the edge of care and edge of custody • Adults out of work or families at risk of financial exclusion • Individuals and families affected by domestic violence and abuse • Abusers of drugs and alcohol • Individuals with a range of (non-age related) health problems • Young people affected by homelessness/rough sleeping In summary, the programme looks at both preventative work to help children, families and vulnerable adults to avoid reaching crisis, and providing crisis management for those that have. Aims and Objectives of IT Solution The Programme is seeking an IT solution that seamlessly publishes information in a secure and “live” delivery to any authorised person, via mobile and agile devices. As we integrate practitioners, teams and management, both within and across other local authorities and local public service partners, at the same time as developing a single multi-agency assessment and service delivery plan; our multi – agency practitioners at all levels will need to be able to interrogate a range of information sources to be able to understand the circumstances and involvements of others. This is both in terms of the ‘here and now’, but also the historic interventions that their predecessors have delivered. By implementing a system that seamlessly publishes that information in a secure and “live” delivery to any authorised person, via mobile and agile devices, for example, a police officer may encounter a person for whom they have a concern, by referring to this source via an existing police device, a clear picture of agency involvements, history and key staff and their contact details can be ascertained. Implementing this facility will “future proof” integration between agencies, enabling the partnership to define and link datasets and views of service users across all partner agencies. Scope The “view” should be set up to access multiple databases holding information e.g. Health records, Children’s Social Care, Early Help Module, Education Management System, Youth Offending……..and the ability to add amend or change the type and number of systems should be unlimited. • A cross index is taken of a person where disparate records are matched across multiple systems. • A multi-agency view of the person across the various databases is displayed, showing basic demographic data from each system, involved professionals, case status, plus any additional key data items. • Detailed information on a person from the source databases is easily accessed via hyperlinks within the Single View interface. Practitioners are provided with an up to date snapshot of the person’s circumstances, by way of the composite chronology of events. • Data can be kept up to date by transferring the correct fields quickly and easily across disparate systems. • Clear alerts ensure users can immediately see discrepancies in a citizens data held on different systems. • One single, intuitive interface enables practitioners to see immediately all the cases they are working on. Social Care’s integration with other agencies plans and theirs with social care create delay, repeated re-referral and duplication in the provision of services. If all of our multiagency front line practitioners could view, whilst out in the field, the range of active and passive agencies who have been involved are currently involved in assessment processes and provision of needs care packages, the whole multi-agency partnership would achieve significant efficiency. Practitioners, commissioners and their managers would benefit from the visibility of service user engagement in other agency plans when they present at our “front door” This project would enable better informed decision making to take place, and increase the signposting to the appropriate service, rather than conducting an expensive social care assessment that reaches the same conclusion. Outline of IT Solution Requirements The IT solution will enable the following: • Portal providing a view of data held in disparate IT systems by different agencies • Initially focused on four multi-agency front doors, both Early Intervention and Social Care where integrated. Later stages of the project will broaden the range of information sources • Information governance is a key theme; the successful solution will demonstrate the ability to meet all IG requirements of the project • The provider may be required to host the solution • Later phases of the project will require a data warehouse solution to enable retrospective reporting and trend analysis The Contract will be used by: Warrington Borough Council, Halton Borough Council, Cheshire West & Chester Council,Cheshire East Council,Cheshire Fire & Rescue Service, Cheshire Constabulary, Homes & Communities Agency Cheshire & Warrington, Public Service Transformation Network, National Probation Service, Cheshire & Greater Manchester Rehabilitation Company, Department for Work & Pensions, NHS West Cheshire Clinical Commissioning Group,NHS South Cheshire Clinical Commissioning Group, NHS Vale Royal Clinical Commissioning Group, NHS Eastern Cheshire Clinical Commissioning Group, NHS Warrington Clinical Commissioning Group, NHS Halton Clinical Commissioning Group, Adactus Housing Group Ltd, Affinity Sutton Homes, Aspire Housing, Frontis Homes Ltd., Torus Housing Group., Great Places Housing Group Liverpool Housing Trust Ltd., Your Housing Group, Muir Group Housing Association, Plus Dane Housing., Sanctuary (North West)., St Vincent’s Housing Association., The Riverside Group., Warrington Housing Association Ltd., Wulvern Housing Association, Equity Housing. It is intended that the initial costs and initial contract will be with Warrington Borough Council as lead authority, however subsequent contracts will be with individual partners of the Programme.
