the overall ethos of this commission will be broken down in to the following two tiers: -
Tier One
The first tier will offer information advice and guidance to support customers into preventative and support services to encourage the adoption of healthier lifestyles and help address health and wellbeing issues. The Commission aims to develop community and individual resilience by promoting self-help and delivering a “step change” in the understanding and take up of preventative services. This includes the promotion and development of existing community voluntary action and self-help groups and support for the development of new groups. The commissioned services will develop their own marketing plans to promote preventative action in a proactive nature across the population and communities and to better understand customer need. This approach will help to increase the individual and communities’ ability to “navigate” preventative and support services and to take that first step into healthier lifestyles (5 ways to wellbeing). Commissioned services will also help to promote streamlined pathways for health and social care needs between services.
The first tier of services will include information, advice and guidance (IAG) and the delivery of a one stop shop service in Darwen and Blackburn Town Centres. The outcome required for this part of the commission is to improve the knowledge, understanding and skills of the community to recognise when preventative help is required and to access preventative services proactively. The focus will be on anticipation, early intervention, prevention and self-management. Services will include support to help people stay healthy, prevent attendance in acute settings and signpost people to low level self-help, early intervention and primary care services. The commission will also support the development of a one stop shop website with links to all health and wellbeing services to help support demand management and flows into other low-level health and wellbeing services (where appropriate) - linking into service mapping work being developed by public health.
All commissioned services will contribute to this website as part of their required marketing strategy – see below. The Single Point of Access will coordinate activity as part of their central role.
Tier Two
The second tier will include action planning for customers with more complex health and social care needs. Customers will be engaged and empowered to develop their individual action/care plans using a strength based approach to build personal resilience and where appropriate, Adverse Childhood Experiences (ACE) informed approach. Commissioned services will utilise the “Star Outcomes” approach to action planning. Star Outcomes will then be used to measure progress to positive outcomes for customers – a key part of the new Outcomes and Performance Management Framework (OPMF). The OPMF will be utilised across the commission as part of a new deliberate approach to measure the achievement of services to deliver outcomes – see below.
The second tier of services will include support for carers and those who are most vulnerable to achieve positive outcomes through the development of an individually tailored personal action/care plan. This will include people in very challenging circumstances to achieve aspirations, older people at risk of losing their independence and support for those with mental health needs. Support is likely to include help with health, housing, jobs, training, social isolation and benefits.
For both tiers the emphasis is on empowerment. The focus of this service is to enable people to build their own resilience and support networks to enable them to be the makers of their own improved outcomes.
