Interested providers are invited to attend a Pre Tender Event with Somerset County Council in Taunton. The event will be an opportunity to hear details about the service that is being commissioned and to ask any questions you may have.
Due to the venue size, we have to restrict the number of representatives attending to one from each organisation.
This notice does not represent a formal commitment by the Council to undertake a procurement procedure.

Somerset County Council is intending to commence a procurement process at the end of July 2016 for a short-term reablement service.
We are seeking interest from organisations that would like to provide these services in Somerset.
The vision for Health and Social Care in Somerset is centred on encouraging people to stay well through a focus on healthy lifestyle choices and self-care. To enable this Somerset is transforming the way that health and social care services work together. This will aim to address the ever increasing demand on services due to demography.
“Reable Somerset”
The emphasis of the Reable Somerset service is to coordinate and join up all care and support, and use local information and resources for successful reablement. The outcome will be helping people in a different way, so that where possible, needs are reduced.
This coordinated approach to care, health and community services allows for good interaction between professionals and people eligible for the service. It will identify local community based and voluntary resources to aid person centred planning and shared decision making. This will build greater resourcefulness within communities with a vision for self-care, prevention and active engagement.
The Reable Somerset service will link with a range of other services being created that offer support for people in their own homes including: Ageing Well, Coast Falls project, Symphony projects in Taunton, South Somerset and Mendip areas.
The Reable Somerset service is short term, and for a fixed period. It will focus on stabilising care plans, designing well-coordinated rotas and supporting smooth transfers to other appropriate services. The service has two key functions:
1. Coordination Role
• Coordinate all care and support services through a single point of access.
• Organise reablement to ensure good timely discharge from hospital, and admission avoidance.
• Liaise closely with health and social care professionals, as well as new and existing services, to design one comprehensive and integrated care plan.
• Adjust and stabilise care and support and source and arrange for any appropriate on-going support.
• Provide a central point of communication for professionals between services about the person.
2. Delivery Role
• Deliver and monitor a reablement care and support service that is regulated to provide personal care.
• Arrange for assessed care and support requirements which complement the care plan and focus on a reducing provision that may include a mixture of initial rota visits.
• Crisis / urgent response provision.
• Stabilise a person to a point where they are able to manage themselves independently or arrange for a smooth transfer to longer-term support services.
