Provision of Targeted Health Checks

Dorset County Council, on behalf of Public Health Dorset, is seeking tenders for a single provider to deliver Targeted Health Checks to specific population groups and to those in priority communities where people are at greater risk of poor cardiovascular health.

The provider of targeted health checks shall:

• Deliver Targeted Health Checks in accordance with NHS Health Check Best Practice Guidance (PHE, March 2016);

• Co-ordinate and manage the service to ensuring effective and efficient engagement, delivery, advice and signposting/referral to onward support;

• Ensure that specific population groups are prioritised;

• Respond to the diverse needs of people in different communities, settings and cultural contexts, ensuring that people who are eligible for a Health Check are given the best possible opportunity to receive one;

• Empower people to take greater control over the factors that affect their health, not only by raising awareness of individual risk, but also by promoting the actions that people can take to improve or maintain their health and reduce or minimise the risk of adverse outcomes from cardio-vascular disease over time.
