Description
To compliment the Integrated Rehabilitation and Reablement Therapy services within Brent, Brent Council is seeking to tender for an integrated rehabilitation and reablement community-based support service.

This new service will work with older people and those with physical disabilities who require short term, intensive assistance to improve independence in daily living skills (e.g. washing, dressing, eating etc.) and to undertake goal orientated reablement activities in their own home.

The service will aim to:
• help individuals to increase independence, reduce their long-term need for care and improve their quality of life.
• work with Health and Social Care professionals to facilitate safe and timely discharge from hospital, assist service users to resettle in the community and promote them regaining independence.

We are preparing to launch the tender in late March, with a view to commence service provision in September 2017.

A briefing session will be held in Boardroom 7 and 8 at Brent Civic Centre on 22nd March from 9.30am to 12.15.

This will be an opportunity for providers to discuss the tender with therapy service and commissioning staff prior to commencement of the formal tender process. We very much look forward to seeing you and discussing the proposals with you.
