CP1341-16 Social Care Assessment Reviews

DCC are seeking to award a contract for the delivery of care reviews and associated administration for Adult Care Operations and Health.
The key strategic priorities are ensuring eligible needs of the person are appropriately met and identify responsible financial savings to the persons care package.
The review process includes but is not limited to:
o Considering information on the person's file;
o Gathering information from other agencies involved with the person;
o Meeting with the person directly and where applicable consulting with their representative;
o Re-assessing the persons needs and eligibility where appropriate;
o Considering personal and community assets that are available to support;
o Evaluating how effective their support has been in achieving the outcomes that are important to the person;
o Exploring options for them becoming more independent;
o Quality Assuring each completed care review;
o Agreeing changes to the care and support plan with the person and/or their representative;
o
o Provider engagement / negotiation;
o Inputting the quality assured care reviews into CareFirst.

The objectives of this contract are:
o To procure suitable resources to successfully conduct 400 priority focussed Medium to High Cost Case reviews in order to realise substantial net cost savings to Devon County Council (DCC) with the first 5 months and potentially more in the future.
o To achieve this whilst working collaboratively with the individual to ensure that the needs of the reviewed person are met.
o To communicate feedback and learning to help improve future DCC processes and procedures.


The contract is 24 Months with the option to extend by a further 2 separate periods of 12 months subject to availability of budget and meeting the objectives of this contract. The contract has a budget for 5 months at £160,000. However after this period there is no guarantee of any further volumes of business or spend.
