Main contract details
Work has begun within Adult Social Care (ASC) in order to provide a more responsive, place and strength based, person centred approach to social work practice and support. A new community social work team has recently been developed (loosely based around the Primary Care Homes across the city) which has quickly started to show an improvement in the length of time people are waiting to be seen and staff continue to work towards practicing in an outcome focussed and asset led way.
ASC continue to build links with local communities, becoming more accessible to those who need advice, information and support quickly. As of October 2019 there are nine Talking Points at a variety of venues around the City where people can see social care staff more quickly, earlier on in the process (either by “dropping in” or by booking an appointment). Talking Points promote an early intervention approach, helping people to access the support they need within their communities, before reaching a crisis situation. Venues so far include a GP surgery, pub, library, and hospital and community hall.
Supporting this transformation, delivery of homecare will be rooted across the same footprints or ‘Neighbourhoods’ – North, East and West with a Key Partner Provider (KPP) for each lot . Our three KPP will work in partnership with the wider ‘system’ to support people who need long term support at home to enjoy healthy, active and independent lives. Provision will be based on enabling people in York to have control over how they manage social care needs with a strong emphasis on the use of personal and community assets and working in partnership.
This will in effect ensure asset based working is a continuum from before the front door, through contact with adult social care and back out into communities for People where long term homecare is needed, effectively turning a linear pathway into a circle rooted in connected communities.
