Description
The contract commences 1 April 2017 for 5 years with the option for up to a 2 year extension.
The service provides alternative placements for service users, some of who may already be placed in secure services out of the borough and prevent the admission of others into out of borough services.
The provider will need to deliver a community based intensive support service that is person centred to individual service user’s needs. The service will have a focus on actively promoting independence pathways that support move on whilst recognising an individual’s risks. The service will respond with appropriate interventions and strategies as part of a multidisciplinary approach in providing positive behaviour support and risk management in line with an individual’s person centred care plan.
This will be achieved through service delivery that promotes a culture of personal growth and development for each individual service user as follows;
• To enable service users to identify their personal goals and have choice and control over how they are supported to achieve them;
• To maintain the self-respect and sense of worth of service users in all situations and their way of life; accepting a wide range of home conditions subject to risk assessment;
• To support service users to maintain and develop skills to live as independently as possible within their own community and to perform functional and meaningful activities with whatever assistance is required. Included in this is the real opportunity to use those skills once acquired;
• To promote community presence though the experience of visiting, belonging, sharing and using the facilities available in the ordinary places that define the local community life;
• To maintain and develop people’s current support networks. The experience of being part of a growing network of personal relationships and the valued contribution which may be made by the individual to these relationships, and to activities that are part of the local community;
• To enable people to participate in their local community and form valued roles and relationships;
• To support people to explore and take up meaningful social, leisure, learning, development and work opportunities;
• To promote a reduction in the need for physical intervention and/or restrictive practice through positive behaviour support and physical intervention care plan.
The service will operate 365 days per year and 24 hours per day where required.
The service is currently delivered across two accommodation sites supporting eleven tenancies provided by separate social landlords as follows;
• Stamford St Stalybridge – 4 Tenancies – Mr Cowell
• Sycamore Court – 7 Tenancies – Creative Support
The commissioners may look to develop the service in the future with a view to widening support for up to thirteen individuals. This may mean the current accommodation will change. The commissioners will be keen to work with the provider in supporting a coordinated approach to any transition to new accommodation.
The service will be delivered within the tendered price in meeting individual’s need and aspirations and this will include the development above to widen support for up to thirteen individuals. There will be an expectation that in delivering the service as outlined in the specification, the provider will work with individuals to build life skills that promote more independence, opportunities to move on to a step down services, less reliance on paid support and therefore gives the provider more flexibility in the ability to meet this requirement.
