Details of the Proposed Service
The Council is consulting on proposals for the tender of the Camden Carers Support Service. The service will provide support to Carers in Camden who provide unpaid care and support to an adult - who looks after a family member, partner or friend who needs help because of their illness, frailty, disability, a mental health problem or an addiction and cannot cope without their support. Supporting carers is a key priority at both a national and local level. Camden has approximately 14,605 residents who provided unpaid care. Working with our partners we take a neighbourhood approach, connecting people to the things that matter to them, supporting carers to have choice and control over the support they receive.
In line with the Care Act 2014 the Council aims to improve health and care support for unpaid carers. This new contract will support Camden’s strategic objective ensuring carers are valued and supported, this is a priority within the Councils Supporting People, Connecting Communities (SPCC) strategy (Supporting people, connecting communities strategic plan - Camden Council').
We are looking for service that will provide:
• Identification of carers.
• Advice, advocacy, information and signposting, including specialist advice for mental health, learning disability, dementia and young adult carers (18-25).
• Partnership working with both statutory and the community sector including working with a range of services to ensures carers statutory needs are met.
• One to one support, support planning and statutory carer assessments / reviews on behalf of the Council.
• Hospital discharge support
• Services that support carers to maintain their health and wellbeing.
• Maintain a Carers database.
• Continued development and support of a carers lead forum.
• Carers breaks/ emergency breaks and activities.
• A digital carers support service
• Carers groups including groups that support ethnic minority communities, Mental Health, Learning Disability and older carers/ Dementia Carers group.
• Carer training sessions, training sessions for our partner organisations/ networking to share knowledge.
• Promote carers services including a range of events either for or about carers.
• Employment support for carers.
• Counselling services.
