This invitation to tender is for the provision of Pathways to Independence (P2i) Targeted Early Intervention, Mediation and Prevention & Transitional and Targeted Pathway Progression Support and Accommodation Service for young people in Somerset.
P2i is an outcome based homelessness prevention service which aims to provide a service for all vulnerable young people aged 16 to 25.
The service aims to address a number of strategic issues with the delivery of youth housing and homelessness prevention services in Somerset with a key focus on:
• Prevention through an early intervention and mediation service to ensure young people are not seeking to leave home inappropriately and that young people understand the realities of living independently.
• Transitions to independence, ensuring that young people are ready and able to live independently, particularly in relation to economic independence.
• A service targeted to those for whom the Local Authority has a statutory responsibility, or for those who would fall into this category if the relevant prevention methods are not implemented.
• An ability to work with young people with the highest level of need and most challenging behaviour in a way that stabilises them and then allows them to prepare for sustainable independence with the best outcomes possible.
The service has been split into the following Lots:
Lot 1: Pathways to Independence Targeted Early Intervention /mediation and Prevention Service (Countywide) for young People in Somerset
Lot 2: Pathways to Independence Transitional and Targeted Pathway Progression Support and Accommodation Service for Taunton Deane and West Somerset
Lot 3: Pathways to Independence Transitional and Targeted Pathway Progression Support and Accommodation Service for Sedgemoor
Lot 4: Pathways to Independence Transitional and Targeted Pathway Progression Support and Accommodation Service for Mendip
Lot 5: Pathways to Independence Transitional and Targeted Pathway Progression Support and Accommodation Service for South Somerset
