Dementia Post Diagnosis Support Service Provider Engagement Event

The London Borough of Bromley and Bromley Clinical Commissioning Group (CCG) is about to embark on the recommissioning of the Dementia Post Diagnosis Support Service (Dementia Hub).

It is a comprehensive support service for people who have been diagnosed with Dementia, their Carers and the professionals who support them.

The Service aims to improve health and wellbeing, reduce and/or delay the need for more costly or intensive services by delivering support in the community, where possible. The Dementia Post Diagnosis Support Service is an established pathway from the point of diagnosis to the point when a person becomes eligible for statutory services.

The Service is jointly commissioned the Council and Bromley CCG. A new contract will be let on 1 July 2020 for a period of 5 years with the option to extend for up to a further two years.

Providers interested in providing the Service are invited to attend a Provider Engagement Event on Thursday 10 October 2019, 9.00am – 12.30pm. The event will be held in Committee Room 1, Bromley Civic Centre, Stockwell Close, Bromley, Kent, BR1 3UH.

The service delivery model will be shaped and finalised through provider engagement. Therefore, the possible service delivery model outlined at the provider engagement event is subject to change.

Due to limited capacity a maximum of TWO representatives will be able to attend per organisation.

The deadline for registering your attendance is on the 9th October 2019 at 4pm. An agenda for the event will be provided to registered participants on the day.
