Framework Agreement for the Provision of Support to Live at Home – Home Care and Extra Care Services

HOME CARE

Homecare is the term used to describe domicillary care services. These services will be commissioned by Medway Council in order the meet the assessed needs and desired outcomes if Service Users. The ultimate aims of the services are to:

I. Ensure that service users remain as independent as possible and providers work in an enabling way that maximises and promotes the service user’s independence and does not foster dependency.
II. Ensure that providers work towards an integrated care system whereby all key stakeholders including but not restricted to service users, service users families, providers, carers, health care professionals, , health care workers, voluntary community services, healthy lifestyle services and social prescribing services work together to achieve the service users desired outcomes
III. Ensure that Social Prescribing Methodology is trained and embedded into care practices to help prevent social isolation and to improve the overall health and wellbeing of the service users.
IV. Ensure that service users are enabled to maintain their independence with appropriate care in their own home

The Homecare Services under this Framework Agreement will be available to adults over the age of eighteen [18] who meet the Council’s eligibility criteria for services.


EXTRA CARE

An Extra Care Scheme is a mixed-use scheme enabling older people with a range of care and support needs to live independently in their own homes. Service users will be fifty five [55] and over including those sensory needs, mental disorder including dementia, short or long term illnesses and those who require end of life care. It develops the concept of housing where older people can have their needs met through the security and privacy of a tenancy but within a socialising environment.

The ultimate aims of the services are to:

I. Ensure that service users remain as independent as possible and providers work in an enabling way that maximises and promotes the service user’s independence and does not foster dependency.

Ensure that providers work towards an integrated care system whereby all key stakeholders including but not restricted to service users, service users families, providers, carers, health care professionals, , health care workers, voluntary community services, healthy lifestyle services and social prescribing services work together to achieve the service users desired outcomes

The Extracare Services under this Framework Agreement will be available to adults over the age of fifty-five [55] who meet the Council’s eligibility criteria for services.


Section 2

For Homecare, Medway Council is seeking to appoint suitably experienced Care Quality Commission (CQC) registered homecare agencies to this Support to Live at Home - Homecare Framework. The Homecare Framework covers the whole of Medway as seen in the Ward Map (Fig 1) and the Ward Data (Table 1) below:

Locality Split

Localites 1 and 2 are both classed as Urban. Locality 3 is split into Urban and rural areas. As such Loclaity 3 is split inot tow catagories as outlined below:. This is explained in the two [2] points below:

I. URBAN – Wards excluding (Cuxton and Halling (Ward 2), Peninsula (Ward 9) and Strood Rural (Ward 18)
II. RURAL – Wards including (Cuxton and Halling (Ward 2), Peninsula (Ward 9) and Strood Rural (Ward 18).

For Extra Care, Medway Council is seeking to appoint suitably experience Care Quality Commission (CQC) registered care provider(s) to delivery services at the following Extra Care schemes.

i. Atlas Place – St. Mary's Island, Medway, ME4 3AN is a thirty-eight [38] unit scheme with twenty-two [22] shared ownership apartments on St Mary’s Island.
ii. Bellerophon House - Doust Way, Rochester, Kent, ME1 1FG is a forty-one [41] unit scheme at Rochester Riverside
iii. Montgomery Court - Choir Close, Wainscott, Rochester ME3 8FP is a sixty-two [62] unit scheme in Wainscott
iv. Prospect Place – 1 Blake Avenue, Gillingham, Kent, ME7 1FX is a sixty [60] unit scheme at Gillingham Pier
v. Rogallo Place – Pilot View, Medway, ME4 6FE is a thirty-eight [38] unit scheme with twenty-two [22] shared ownership apartments in Horsted, Rochester

A full and detailed Service Specification (Appendix 2.1) is attached to the competition documentation. Specific requirements for the five [5] schemes are also included as Appendices 2.2, 2.3, 2.4 ,2.5 and 2.6 of the Tender documents.
