Lot 1. 18 unit accommodation based support service with nighttime concierge
The Service will provide support for adults with mental health issues who have eligible social care needs and the placement forms part of a Community Mental Health Team (CMHT) Care Plan. It will be delivered within a purpose built, apartment block close to the city centre. As part of the Service the Provider will be required to offer good quality, responsive housing management on behalf of the property owner. The Provider will be responsible for the day to day management of the building through a formal Managing Agent Contract with the property owner.

Lot 2 20 unit accommodation based support service with nighttime concierge
The Service will provide support for adults with mental health issues who have eligible social care needs and the placement forms part of a Community Mental Health Team (CMHT) Care Plan. It will be delivered within a purpose built, apartment block close in the south east of the city. As part of the Service the Provider will be required to offer good quality, responsive housing management on behalf of the property owner. The Provider will be responsible for the day to day management of the building through a formal Managing Agent Contract with the property owner.

Lot 3 Assertive prevention support service
The Service will provide assertive prevention support to clients living in their own homes in all tenures to develop resilience and remain independent in the community. The service will prevent people from needing adult social care, prevent people with ill health from getting worse and preventing people who have needed care from needing it again and supporting them in their recovery journey The Service will also proactively work to support people leaving hospital, residential care, mental health supported housing and support their recovery and independence.

Lot 4 Dispersed supported accommodation
The Service will provide support for 25 adults/households with mental health issues living in dispersed accommodation in Sheffield. The Service will take a proactive and assertive role in steering a journey to recovery and reduce dependence on paid support. It will prevent people: • from needing adult social care • with ill health from getting worse • who have previously had care from needing it again. This lot requires the provider to bring some units of accommodation with them

Lot 5 PIE. Psychology service
The service provider will deliver advice, consultation, supervision, case formulation for behaviour management to providers of housing related support, in order to embed psychologically informed practice in the service. It will also provide some direct therapeutic support to some female residents who cannot access mainstream therapeutic service.
