Description:
Medway Council and Medway CCG are looking for a lead provider who can work with us over a 5 -7 year period to deliver our commissioning model to implement the Medway Intermediate Care and Reablement Strategy. This model is described within the Memorandum of Information on the medway Portal and was covered and discussed in a fuller manner at a market engagement event on 26 November 2015. Working with other Providers in Medway the lead provider will oversee the whole range of services, sub-contracting for some of these services. The lead provider will work with commissioners and a provider consortium to ensure the most appropriate care is in the right place at the right time for all patients and service users in Medway who need intermediate care services. Over time a shift in the pattern of services in Medway will occur so that more people are supported in their own homes and less in beds. The services to be provided will be safe, effective, personalised and fair services to patients and service users with health and social care needs that require a range of Intermediate Care services in order to be supported to remain in their own homes. To date these are bedded and non- bedded services commissioned separately by Health and Social Care. We are developing a more flexible service so that care can be deployed to the most appropriate environment which will include peoples’ homes, nursing or residential homes. This will be a joint service provided as part of the Medway Better Care arrangements and the funding is part of a pooled budget between the Council and the Medway CCG. The service will be provided in accordance with the statutory responsibilities of the Council and CCG and will be managed as part of the sections 75 partnership agreement between the Council and CCG.
