NAME CHANGE TO GOODSHAPE AND CONTRACT PROVISION UNTIL 2026 This contract was approved by EMG at 25th July 2022 to be exended for a further 12 months until 30/9/23 - agreement to be signed asap and added to this register Ref Service Deliverable 1 A dedicated 03 absence reporting phone line for exclusive use by employees of The Customer to report, update and close absences. 2 24/7 Recording of Unplanned absences, including Absence Start Date, Return to Work Date, Time Lost, Absence Reason, Fit Notes (if applicable), Contact Telephone Number and any additional information the absent employee wishes to relay. 3 The signposting of absent employees to other support services, such as Occupational Health, Employee Assistance Programmes and Physiotherapy (where available). 4 24/7 distribution of email and SMS notifications to recipients designated by The Customer, following the reporting of an absence by an employee of The Customer. 5 Tracking of line manager Return to Work Interview completion against agreed target and the management of customisable interview forms. 6 Real-time dashboards, showing all active absences, outstanding actions, recent alerts, high absence areas and key absence reasons. 7 Absence Archives showing the details of all historic absences, configurable by user preferences. 8 Employee Reports, providing a complete overview of the absence history of a chosen employee of The Customer, including the details of every absence, statistical analysis, completed return to work interview forms, notes, alerts, referrals and employment details. 9 Focus Lists, providing a summary of the employees that have had the highest amount of absence. 10 Compliance Reports, providing detailed and trend analysis of line manager compliance with the Return to Work Interview process. 11 Absence Reason Trends, showing days lost, absence spells and average length of all reasons over set time periods, based on user preferences. 12 Absence Rate Trends, showing the absence %, days lost, absence spells and average length of absence over set time periods, based on user preferences. 13 Payroll Export reports listing the details of all absences (as defined in Schedule 5). 14 Account Management support in the analysis of absence trends and management of the service. 15 24/7 Medical Primary Care Advice for all employees reporting medical absences, the goal of which is to help employees of The Customer to return to health, and to work, as early as practicable.
