Description:
Lot 1 - Group provision and outreach provision
a) Group provision - Offering a range of group activities for children and young people, which includes opportunities for social inclusion and enrichment within the community.
b) Outreach - Offering support to children and young people within the community (or home setting, if required) to work towards outcomes identified within their care plan.
Lot 2 -Holiday provision - Offering a range of group-based activities or 1:1 provision for children and young people, within the school holidays. This could include a buildings-based activity, or an activity within the community.
Lot 3 - Provision of personal care within the child’s home - Offering personal care support to children and young people.
Lot 4- Provision of day opportunities for children aged 16+ - Offering young people opportunities to promote social and independence skills to meet outcomes as identified in their transition care plan.
Lot 5 - Provision of intensive short breaks - Offering intensive support to children and young people where needs escalate; this may be short term or longer-term support.
Lot 6 - Provision of outreach services (where main provision is unable to meet demand, or family choice) - Offering a range of options for children and young people to access individual or group support within the community to meet the needs identified within their assessment.
Description of the geographical coverage:
It is expected that service delivery is undertaken using the facilities within the Borough but there may be instances where activities and opportunities outside of the Borough would support the outcomes in the child’s care plan. Where activities are provided outside of the Borough, it is expected that families will fund the additional costs. This will be discussed and agreed with family members.
